Please accept my entry into the Madonna University Crusader 5K/Madonna Mile run/walk. I state that I am physically fit for the event. I agree to the 30-minute per mile pace maximum course time limit and understand there is no course support after one hour and 45 minutes past the start time. I waive on behalf of myself and my heirs any rights I may have against Madonna University, its employees, volunteers, consultants or sponsors for damages or injuries occasioned by my participation in this event. I also authorize Madonna University to utilize my photographs, personal narrative, interviews or video recording of my participation in the Crusader 5K/Madonna Mile run/walk for any and all purposes.
