I hereby consent to Madonna University using any film, photograph, audio or video recording, printed material, and/or statements made by me (or the subject named below) without compensation. The material obtained through this testimony, photography, and/or recording may be used by Madonna University and its authorized partners for educational, promotional, or marketing purposes. This includes, but is not limited to, use in print, broadcast, online, and social media, or any other medium used to promote Madonna University and its programs.

I confirm that I am at least 18 years old and legally able to sign this release, or that I have obtained explicit written consent from a parent or legal guardian (whose information is provided below).

I understand that this agreement is legally binding and that I am signing it voluntarily.
